To request a Middle School Visit as part of our MVP Program, please submit the following survey, estimating information to the best of your ability. Due to the limited number of program dates, requests will be addressed in order of submission.  Middle school visits occur on most Tuesdays and Thursdays with occasional Monday, Wednesday, Friday availability.
